Community Health Nursing Across the Lifespan

• Standard VII: NUR 3636 Community Health: Integrate principles of disease prevention and health promotion in providing care to individuals, families, groups, and communities

This class focused on the care of patients of all ages in the local area as well as in state, national, and international communities. I learned how to advocate good health practices to prevent disease in community health through the lens of health care disparities. A windshield  evaluation of a local community was completed to seek a public health problem that should be addressed. The first page in this file contains a form that was completed to assess a local community to determine potential health problems. The second page is a written reflection of the assessment and the identification of a community health issue that was discovered and potential solutions to the problem.
